LAM Reseller Credit Application (fill out our interactive PDF form online, print it out, sign it, and either fax or mail the document to us)

Sales Tax Certificate (Forward certificate with other enrollment documents)

LAM Reseller Credit Card Authorization Form (For credit card purchases, complete and return with other enrollment documents)
